Kenneth W. Deeb, M.D., FACEP
Dr. Deeb graduated and completed post-graduate training from the University of Louisville School of Medicine in Louisville, Kentucky. He has been engaged in the full time practice of Emergency Medicine for over 25 years, having been transferred to the DFW Metroplex while in an administrative post in 1990. Dr. Deeb is Board Certified in Emergency Medicine and a Fellow of the American College of Emergency Physicians. In addition, he has been certified multiple times as an Instructor and Provider in Advanced Cardiac Life Support as well as being certified in Pediatric Advanced Life Support and Advanced Trauma Life Support. In addition to these clinical credentials he has served in two senior administrative rolls in the Emergency Medical Field, ten years as a Regional Medical Director and later as a Regional Vice President of Medical Affairs for Spectrum Healthcare Services, a wholly owned subsidiary of ARAMark. Most recently he served as Regional Medical Director to the Southwest Region of EmCare, another large Dallas based provider of Emergency Services to hospitals throughout the US, before joining the First Choice family over 3 years ago. Dr. Deeb resides in Denton County with his wife, Colleen and two of their children.

Gregory Blomquist, M.D., Area Medical Director, DFW
Dr. Blomquist received his medical degree from Chicago Medical School in 1991. He completed his residency at UT Southwestern, Parkland Memorial Hospital where he remained on staff as an instructor until 1997 and practiced in area hospital emergency departments. He is board certified in both Internal Medicine and Emergency Medicine. Dr. Blomquist lives in Plano with his wife and two children.

Dr. Muzzarelli received his medical degree from the University of Texas Southwestern Medical Center at Dallas in 1998. In 2001 he completed his residency at Wayne State University/Detroit Receiving Hospital in Detroit, Michigan where he served as Chief Resident. He then returned to North Texas as Attending Emergency Room Physician at Medical Center of Lewisville until he joined Dr. Novak at Flower Mound Emergency Care Center in 2004. Dr. Muzzarelli and his wife live in the Flower Mound area with their two children.

Dr. Rab received his medical degree from the University of Texas Medical Branch in 1982. He did his emergency training at the University of Arkansas/Little Rock. He practiced emergency medicine in area hospitals for 20 years until he joined First Choice Emergency Room in 2004. He is board certified in emergency medicine. Dr. Rab and his wife live in Dallas and enjoy spending time with their two grown daughters.

Juan Nieto, M.D.
Dr. Nieto received his medical training at the University of Colorado Medical Center in Devner in 1974. He finished his residency in Emergency Medicine in Denver. He has practiced in the Austin area for over 25 years. During this time he has been the medical diector of the Brackenridge emergency department where he was involved in planning Starflight. He has also practiced in the private sector and, for the past 16 years, has had an academic appointment in Emergency Medicine at the UT Health Science Center in San Antonio. He is Board Certified in Emergency Medicine and has received numerous awards by professional associations. His interests include volunteering in local clinics and on international medical missions. He enjoys discovering the vast Big Bend area with his camera and his two daughters.
First Choice Emergency Room & Medicare
Currently, Medicare does not recognize free-standing emergency rooms for reimbursement. FCER has worked for the last few years to help shape recent legislation, creating licensing standards for free-standing emergency rooms in Texas. We hope that this recognition will help facilitate the process of facilities like ours being recognized, and eventually reimbursed, by Medicare.
Medicare Patients
If you have private insurance - your services will be covered by your private insurance and you'll be responsible for paying any copayments or deductibles, at standard ER rates, just as if you visited a hospital-based ER. If you have Medicare as secondary insurance, FCER cannot file a claim for expenses not covered by your private insurance, because Medicare does not cover services provided by FCER.
If you are covered by Medicare, Medicare Replacement, or Medicare with Supplemental - you will be responsible for paying for your treatment in full at the time of service. Medicare does not cover services provided by FCER, and will not reimburse you for these fees.
We have a three-tier prompt pay pricing structure for your convenience. This system is based on procedures performed, and will be determined once you have been evaluated by a physician. The three fee levels are as follows: $225, $475, and $925.

Andrew Kretschmer, M.D.
Dr. Kretschmer received his medical degree from The University of Texas Medical Branch in 1984, After residency time in family practice and anesthesiology he went on to become board certified in emergency medicine. Dr. Kretschmer has been emergency room medical director at Doctor's Hospital in Conroe, Woodland's Community Hospital, and Tomball Regional Hospital. Currently he is the medical director of Northwest EMS, voted best EMS in Texas in 2005. He and his wife Nancy have lived in the Woodlands, with their 3 children, since 1984.

Oscar Vela, MD
Dr. Vela earned his medical degree at the University of Texas Health Science Center in Houston. He trained at Texas Tech Universitys Department of Emergency Medicine. He has special interest in emergency radiology and academic medicine. He serves as a clinical instructor at Texas Tech Health Science Center in El Paso. Dr. Vela is board eligible with the American Board of Emergency Medicine. He is fluent both in English and Spanish. He travels within Texas to work in trauma centers in the Corpus Christi and El Paso area.

Dr. Olson received his medical degree from the University of Nebraska, and then completed his Emergency Medicine residency at the Medical College of Wisconsin in 1983. While in Milwaukee, he worked in a private practice, and then served as the Director for the Emergency Medicine residency. After moving to Texas, he was the Chairman of Emergency Medicine at UT Houston Medical School, and then became president of the emergency medicine physician group at Northeast Medical Center in Humble, before joining First Choice ER in 2007. He and his wife, Debbie, have raised 4 children.

Dr. Mitchell received his B.A in Biochemistry at California State University Fresno, as Dean's and President's medalist in 1998. He was a 4 time WAC track and field champion and GTE Acadamic All American. He received his medical degree from Johns Hopkins University School of Medicine in 2002. He also completed his internship and residency in Emergency Medicine at Johns Hopkins Hospital in 2005. He is board certified in Emergency Medicine. Dr. Mitchell and his wife Kisha live in The Woodlands, with their 4 children.
